쉽게 볼 수 있는 증례부터, 해결되지 않았던 증례들까지 다양하게 준비한 콘텐츠들을 보시고 진료현장에서 적용하는 데 많은 도움이 되시기를 바랍니다.

■ soft tissue tumor , palm, Lt

■ 성별/ 나이 :M /49

■ 병력
* 수년전 부터의 왼쪽 손 바닥 통증
* 엄지와 검지 사이 웹 통증
* 내부의 이물감, 덩어리 촉지, 압통
* 가끔 자발적인 통증이 심해짐
* 물건 쥐기 힘들 정도로 통증이 올때도 있음
* 타 병원에서 석회화 병소 발견됨
* 충격파 주사 치료 등의 치료 호전 별로 없음

■ 이학적검사
* Swelling of the Lt. 1st volar web space
* Hard but movable mass occupying the entire web space
* Tenderness, marked

■ Impression
*Hemangioma
*Giant cell tumor

■ Treatment
*Referral for surgical excision

Long axis scan along Lt. adductor pollicis m.

1. Adductor pollicis t.
2. 1st interossei mm.
3. 2nd finger flexor t.
4. Lumbrical m.
MC. Metacarpal bones

Lt. adductor pllicis muscle is diffusely swollen and mixed-echo mass is seen, but not clearly demarcated.

Compared to the Rt. Side, mixed echogenic lesion is located within Lt. adductor pollicis muscle(1) and shows high echoic change(*) under the lubrical muscle(L).

Long axis scan along Lt. adductor pollicis m.

Power Doppler image shows mild hypervascualarity within the mass. Small calcified foci(arrows) are seen.

Within Lt. adductor pollicis muscle, irregular shaped high echoic lesion accompanying mild hypervascularity is seen

Upper

T2 weighted transverse MRI scan shows oval-shaped well demarcated high signal mass is occupying the Lt. adductor pollicis muscle. The mass encases over the half of the 2nd flexor tendons.

Lower

T1 weighted FS and contrast-enhanced transverse MRI scan shows the mass shows irregular contrast enhancement pattern.
